Requirements for Enrollment

-At the time of registration, parents or legal guardian must provide the following documents.

- Withdrawal form from last school

- Birth Certificate

- Child's Social Security Card

- Alabama Blue Immunization Form

- Any two of the following documents for address verification:

    -- Mortgage or lease (rental) agreement

    -- Copy of any recent utility bill

    -- Copy of property tax records

    -- Automobile registration

    -- A court decree declaring the resident as the legal guardian
